1. Understanding your situation
An integrated, objective and professional overview can assist you and your family in identifying primary issues and goals. Sage Eldercare Solutions' geriatric care managers start by conducting a comprehensive assessment that gives them insight into the client's health, functional, emotional, psychological, legal and financial status as well as their individual needs and interests. Most important, our team members recognize that taking this first step can be difficult and work with you and your family members in a caring and compassionate way.

2. Development of a comprehensive care plan
From this multi-faceted evaluation, we are able to create a written care plan that addresses the present concerns with actionable steps to improve the health and safety of the older adult. We encourage family members to review the care plan and provide input regarding the recommendations.

3. Professional Consultation
An older adult or their family members may choose to engage the care manager for consultation after an initial assessment. The care manager presents options and guides the planning and decision-making processes on an as-needed basis. Sage Eldercare Solutions' Professional Consultation services include long-term care planning sessions for those who would like to proactively develop a plan for long term care.

4. Implementing the Care Plan
Sage Eldercare care managers are available to implement the plan in conjunction with family members. Implementation may involve:

  • Securing qualified and trained home care
  • Managing medications to ensure compliance
  • Health advocacy and professional assistance with medical appointments
  • Assessing appropriate residential care alternatives and planning for the next steps
  • Arranging for meals, housekeeping, bill paying, home organizing, transportation, or other services
5. Monitoring and Ongoing Re-assessment
Clients may retain Sage Eldercare services for a given period of time or indefinitely to ensure continuity of care and services. Care managers provide on-site coordination, home care supervision, and continuous advocacy. In proactively planning for future needs, we help create security, comfort and peace of mind knowing that the Sage Eldercare team members are in place to handle any crises or changes in condition.

6. Monthly Reporting
We take great pride in delivering a detailed invoice to each client every month reporting the service we delivered in the previous month, the date of each action, time elapsed, fee, and a description capturing the results of the action. We have found these detailed invoices are the most effective way to provide family members (particularly those living at a distance) and fiduciaries the best understanding of the goals and outcomes of each action taken on the client's behalf.
