Sage Eldercare Solutions offers adult children, family members, and conservators "care management clinics" - 60 minute, personal sessions with a professional care manager to discuss eldercare issues and solutions. Your personal care management session can:

• Address your eldercare questions or concerns and provide solutions - such as trusted service providers, resources to call, next steps to take, warning signs to watch.
• Implement an Aging in Place Decision Model to evaluate needs, costs, services, family support, and other considerations of aging in place - remaining in one's home for as long as possible.
• Initiate in-home help or other services, including screening, arranging, and monitoring assistance in hiring a qualified caregiver for home care.
• Facilitate planning for a hospital discharge or other medically complex situation
• Provide short- or long-term eldercare assistance for those engaged in local or long distance caregiving.
• Review financial, legal, or medical issues and offer referrals to geriatric specialists.
• Provide crisis intervention.
• Act as a liaison to families at a distance, overseeing care, and quickly alerting families to problems - especially important when families are engaged in long distance caregiving for a loved one.
