Adrienne Uyenco
Care Manager
MA Counseling Psychology, Santa Clara University
Adrienne is a devoted advocate and feels passionate about working with the elderly population. She has worked as a Social Services Director in various skilled nursing and rehabilitation facilities in the Bay Area, gaining much experience working with clients with Dementia and complex medical conditions. In those roles, her responsibilities included facilitating clients' return home with care, conducting family meetings to educate family members about a loved ones needs' and assist in decision making as well as connecting clients and their family members with resources for support, such as support groups and adult day programs. Most recently, Adrienne has worked as a Case Manager for persons with Acquired Brain Injury residing in a supported living and rehabilitation facility. She earned a Masters Degree in Counseling Psychology with an emphasis in Health Psychology from Santa Clara University. Additionally, she completed her Counseling practicum doing grief counseling in the bereavement department through hospice.
Angela Berger

Angela graduated with Latin Honors and has worked with older adults as a volunteer for the Veterans Administration. Most recently, she was a technician working with a veterinary surgical center.
Ariana Alex, MA
Care Manager

Ariana has conducted intergenerational research with the San Francisco FROMM Institute for continued learning as well as volunteering at Central Gardens SNF. Ariana has also worked at Coventry Park Assisted Living Community in San Francisco for over four years as the Director of The Memory Enhancement Program as well the added role of Associate Assisted Living Director. She also has direct experience in creating and conducting activities for older adults, especially those with memory loss. Ariana attended San Francisco State University where she received her Master's Degree in Gerontology focusing in Long-Term Care Administration.
Barbara Gignoux, RN

Barbara has had a wide range of experience in the nursing field, including her most recent role as the nursing supervisor for ResCare, which provides residential care for developmentally disabled adults. Prior to her role at ResCare, she was the Clinical Coordinator of Pediatric HIV and AIDS Program for Monmouth and Ocean Counties in New Jersey. For her work in this field, Barbara received the Starfish Society Award, and her biography appeared in Who's Who in America, Who's Who of American Women and Who's Who in American Nursing.
Claudette deCarbonel, PhD, MFT

Claudette is a licensed Marriage and Family therapist with 12 years experience working in community behavioral health serving many different populations including the elderly. Her interest in working with the geriatric population includes a long relationship and personal involvement as a volunteer with seniors in the community as a visitor, family member and board member and her own personal experience as a caregiver. She is family systems therapist and recently completed her dissertation focusing on applications of family systems theory in the workplace.
Diane Loeb, MSW, LCSW

Diane is licensed by California’s Board of Behavioral Sciences as a Licensed Clinical Social Worker. A geriatric specialist, Diane advised and assisted thousands of older adults and their families during her 30 year social work career at San Francisco’s Jewish Home. Her positions within that organization included: Intake Specialist, Outreach Coordinator, Director of Social Services, Director of Admissions and clinical supervisor to dozens of graduate school interns. An expressive artist, Diane is co-founder of ArtSpark, an art therapy practice that brings art and recreational services to the homebound. In addition, Diane serves on the boards of Bay Area Social Workers in Health Care and In-Home Supportive Services Consortium.
Jack Herndon, MBA

Jack has over 20 years of experience in marketing, management and business development roles in companies ranging from Fortune 1000 organizations to start up organizations (for profit and not-for-profit). He has co-founded two companies and has raised more than $22 million in venture capital funding. He and his wife Nina own and manage Sage Eldercare Solutions (www.SageEldercare.com), a care management practice which includes nurses, social workers and gerontologists. In addition, Jack currently teaches care mangers thru the University of Florida certificate program (Ethical, Legal and Business Aspects of GCM) and is co-author of the forthcoming chapter in the Geriatric Care Manager Handbook "Adding GCM to a Home Care Agency". He is a founding board member and former Treasurer of the San Francisco Village (www.SFVillage.org). He recently published The Hospital Discharge Organizer and has led numerous workshops for families navigating the eldercare maze. Jack earned his B.A. from Brown University and his MBA from Stanford University.
Janeane Randolph, MSW

Janeane has had the good fortune to work professionally with older adults and their families for over 25 years. Janeane earned her Master's Degree in Social Welfare, with a concentration in gerontology, from the University of California at Berkeley. After graduate school she developed a care management program at Catholic Charities for older adults and then led the development and operation of an adult day health care center sponsored by St. Mary's Hospital and Medical Center. At California Pacific Medical Center, she was the Administrator and Executive Director of Senior Services. She also served as a founding member of San Francisco's Elder Abuse Prevention Task Force and a Board member of the San Francisco Adult Day Services Network. Prior to joining Sage Eldercare Solutions, Janeane was the founding President and CEO of Elder Care Alliance.
Laura Callagy, MA

Laura has been involved in the elder care industry for over 20 years. She has worked for the Federal Government in the policy department for the Center for Medicare and Medicaid services as well as serving as a bilingual claims representative with the Social Security Administration. Laura worked for a Peninsula Adult Day Health Program in the Activities Department and as the transportation coordinator. Laura attended Santa Clara University and received her Masters degree in Gerontology from San Francisco State University.
Mary Collins, MSW, LCSW

Mary is a Licensed Clinical Social Worker with over 10 years of experience working with older adults. After receiving her MSW, Mary worked as a Case Manager at Curry Senior Center in San Francisco where she provided long term case management and caregiver supervision for her clients. Mary has also worked for the Department of Aging and Adult Services with the City and County of San Francisco in both the Public Guardian/Conservatorship office and with Adult Protective Services. Mary has focused her career on providing older adults with services and support systems that will enable them to maintain their highest level of independence and quality of life.
Michelle Leung

Michelle brings over 10 years experience as an office manager for various health care offices. She has been involved in many community outreach programs and was awarded a Certificate of Recognition by the California State Assembly for her volunteer work.

After becoming vice president of a major public relations firm's health care division, Nina chose to earn her Master's degree in Gerontology and work with aging adults as a geriatric care manager. After receiving her graduate degree, Nina worked with several home care and care management agencies in the Bay Area. Nina founded Sage Eldercare Solutions in 2001 to enable people to make wise eldercare decisions, relieve caregiver worries and help older adults live longer, happier, and healthier lives. Nina is a Certified Care Manager and a member of the National Association of Professional Geriatric Care Managers and was recently named Outstanding Member for the Western Region of the association.
