MSW, RN, MFT - Care Manager Position Available

Sage Eldercare Solutions is looking for enthusiastic, detail-oriented and skilled professionals who have a can-do attitude to join our interdisciplinary team of care managers in a stimulating work environment. We are hiring for both full and part time positions, and we offer compensation at the top of the industry. Sage Eldercare Solutions is a growing practice in San Francisco, Marin and San Mateo counties, dedicated to offering wise eldercare solutions that promote quality of care and quality of life for older adults.

As a Sage care manager, you will work with older adults and their families to: conduct comprehensive assessments; develop and implement individualized care plans based on the older adults' specific needs; communicate with clients and their family members, doctors and medical staff; and monitor clients' situation on an ongoing basis to ensure changing needs are addressed.

Responsibilities:

• Conduct in-home assessments and draft written summaries of findings
• Develop written care plans and implement various components of care plan
• Negotiate family dynamics to agree on and implement the care plan
• Cultivate and maintain extensive knowledge of community resources available to older adults
• Provide ongoing training and supervision of in-home caregivers
• Communicate regularly with family members and appropriate professionals
• Collaborate with professionals and advocate for clients' needs

• MSW, RN, MFT, or MA Gerontology
• Experience in care or case management
• Availability all weekdays and some evening/weekend on call rotation
• Knowledge of local eldercare resources (preferred)
• Excellent communication skills (written and oral)
• Proficiency in Microsoft Word, Excel, Outlook
• Valid California driver's license, insurance, car
